- Fatty liver usually causes no symptoms at all.
- The real question is whether there is inflammation or scarring.

What the report is actually saying
An ultrasound can see fat in the liver because fat reflects sound differently from normal tissue. “Grade I, II or III” describes how bright the liver looks, which roughly tracks how much fat is there. That is all. The scan cannot see inflammation, and it cannot measure scarring well.
So a fatty liver report is a starting point. In most people the fat sits there quietly for years. In some, it triggers inflammation, and over a long time that inflammation leaves scars. Working out which group you are in is the point of the follow-up.
Four questions to ask
First: do my blood tests show inflammation? ALT and AST are the usual markers. Second: should I have a FibroScan to check for scarring? Third: what is driving this in my case, is it weight, sugar, cholesterol, a medicine? Fourth: what target are we aiming for, and when do we check again?
What helps, and what does not
Losing 7 to 10% of your body weight over several months reduces liver fat and inflammation in most people. The method matters less than sticking with it: smaller portions, fewer sugary drinks, a brisk walk most days. Treating diabetes and high cholesterol properly helps the liver too.
What does not help: liver tonics, detox teas and supplements sold as cures. Some of them actually injure the liver. If a product promises to clean your liver, keep your money.
